Welcome to Summer Adventures 2011! Please read through the general information packet (download at right) carefully as it will help answer many of your general camp questions; however, please feel free to notify the camp office should you have any further questions. FORMS TO BE FILLED OUT AND RETURNED TO CAMP OFFICE ON OR BEFORE THE FIRST DAY YOUR CHILD COMES TO CAMP. PDFs are available to download in the box at right.

Authorization to Release Form: This form tells us who may and may not pick-up your child. We will not release your child to anyone other than a parent/guardian unless that person is designated on this form. Please understand this is for your child’s safety.

Acknowledgment Form for Internet Use

Authorization for Medication: This form is required for all campers who will be taking medication during their time at our camp. This form must be signed, brought to the nurse on the first day of your child’s camp session along with the medication in its original packaging. Any medication not in the original packaging cannot be accepted.

Permission Slip for Patriots Baseball Game: This form is required for campers in Grades 4-5 Day Camp and all Extreme Sports participants registered during the week of August 1.
